We accept most insurance companies. Please make sure that we always have the most current insurance information on file for all of your children, by allowing us to copy your insurance card at your visit. We will file insurance claims on your behalf. The patient/parent is responsible for anything that is not covered, determined to be not eligible or for any other reason the claim comes back unpaid. All co-payments must be paid at time of service.

Also, please remember that you have 30 days in order to add your newborn to your insurance policy. Check with your insurance carrier or your Human Resource Department for details on how to add your baby.

We belong to two Independent Physician Associations (IPA) for the BCBS of Illinois HMO. You may choose either ShermanChoice (site 359) or Northwest Suburban IPA (site 197). ShermanChoice utilizes Sherman Hospital in Elgin and Northwest Suburban IPA utilizes St. Alexius Medical Center in Hoffman Estates. Contact BCBS for further information.
