Office Hours - Monday-Friday, 8:00AM to 5:00PM and Saturday from 8:00AM to 12:000PM. Evening hours are available through out the week. Appointments start at 9:00AM with the physicians. Please call after 8:00AM for an appointment.
Routine Well Visits are scheduled from 9:00AM to 4:30PM, Mondays-Friday, and 9:00AM to12:00PM on Saturdays. Well child care visits are scheduled according to the guidelines of the American Academy of Pediatrics.
Prenatal Consultations are welcome as an introduction to our practice. Consultations may be scheduled upon request and are free of charge.
Sick Visits – If your child needs to be seen because of an illness, same day appointments during office hours are always available. Please call to schedule an appointment. If your primary physician is booked or out of the office, you will be able to see another physician or nurse practitioner in the practice. If you schedule an appointment for an illness, please note that a full checkup cannot be done at that appointment.
Refills - Please call for prescription refills Monday-Friday during regular business hours. Prescriptions will not be refilled by the on call physician.
Late For An Appointment – We realize that some days do not always run as smoothly as planned. If you are late for an appointment, we will try to fit you in for the next available appointment, schedule permitting or your appointment may be rescheduled.
No Show Policy - Quality care for our patients is our priority. Please take a few minutes to review our no show policy and sign at the bottom of the form. If you have any questions please let us know.
We do understand there could be a circumstance when you are unable to arrive for appointments. However, if this becomes a pattern for your family’s appointments, there will be consequences involved. If you have missed an appointment in our office we ask you to be sure to keep future appointments or cancel them with 24 hours advanced notice to avoid being considered a “no show”. Last minute cancellations still result in lost appointment times. Appointments made for the same day you call will require at least two hours notice of cancellation.
All “No Shows or Late Cancellations are subject to a $25.00 fee. These fees are not covered by insurance companies and will be the responsibility of the guarantor.
Associates in Pediatrics will attempt to contact you 24 hours prior to your scheduled appointment to confirm your visit. *Please remember confirmation calls are a courtesy, ultimately it is your responsibility to know your appointment date and time.*
If your family has had a total of three no show appointments within a calendar year, or an excessive amount compounding over the years, you may be asked to find another practice that better meets the needs of your schedule
Payment Policy - Payment for services is expected at the time of your child’s visit. This includes co-pays, balance from prior visits and payment in full when we are not contracted with your insurance carrier. In accordance with the terms of our contract with your insurance company, we are legally required to collect all co-pays from you at the time of service. We accept cash, checks and credit cards. The accompanying adult or parent is responsible for full payment and providing current insurance information.
Consent For Treatment Without A Parent Present - Associates in Pediatrics will medically evaluate and treat your child without a parent present. However, a consent form signed by the parent will be required for a minor not accompanied by the parent at the time of the visit
Referrals To Specialists – For HMO Patients referrals must be requested at least 5 days prior to your appointment, unless the visit is an emergency. Referrals can be faxed directly to the specialist or picked up at our office. If you do not request a referral prior to seeing a specialist, we cannot backdate a referral. This violates our contract with the insurance company. Without a valid referral you may be responsible for charges incurred at the specialist’s office.
